The more you practice and work at it the more you'll find that what you play will fit. Knowing where you are in the piece will also get better and you'll find you won't be thinking about it - your ears will be telling your brain and your brain will be telling your fingers and often you won't notice ...

I think your problem might be lessened wth a concentration of more hammer ons and pull offs and slides which should help the flow.
